The following documents must be provided by the parent or legal guardian
at the in-person enrollment appointment:
Birth Certificate or passport
Social Security card
Immunization records
Proof of residency (deed, mortgage, lease, pay-stub, current utility bill, or mail showing you are residing at the address listed).
Parent/Guardian’s driver’s license or state I.D. - this will not be accepted as proof of residence.
Custody papers – if applicable
*If custody is in process, the parent has 60 days to complete.
*A letter from an attorney or court must show status of the case at enrollment.
Grade card or transcript from student’s previous school attended
Copy of ETR/IEP if applicable
You may upload any of the required documents digitally in the FinalForms application or bring them with you at your appointment time.
Before upload, please label your documents with child’s name
